Google’s Pixel Watch 5 is less about a new-looking device and more about what the watch can do once it is on your wrist. The hardware changes are limited, but the software story is much broader: more Gemini, more proactive alerts, and more health signals that try to turn passive tracking into useful context.
The watch costs $399, which is a $50 increase from last year. It also brings a new satin pyrite case finish, several new strap colors, a Steph Curry Special Edition, a slightly faster Qualcomm processor and a small battery increase. Those updates matter, but they are not the center of this release.
Software carries the Pixel Watch 5 update
The clearest shift is that Google is treating the Pixel Watch as a place where AI should be more present and more immediate. Gemini can now handle simple requests offline, including starting timers and opening apps. In the demo described in the source article, offline use was noticeably slower, but the point is still important: some basic assistant tasks are no longer fully dependent on a live connection.
Gemini is also becoming more proactive. One example shown involved a text asking for availability, after which the watch offered a way to check Google Calendar. That is a small interaction, but it shows the direction Google is taking: the watch is meant to surface the next likely action instead of waiting for the user to dig through menus.
There is also a new watchface icon that can show real-time information, including timers, Uber ETAs and workouts. Separately, the pinchy pinch and wrist-flick gestures that arrived through a software update to Pixel Watches in December are more deeply built into the interface with WearOS 7.
Google is even letting users generate watchfaces with AI. In one example, the prompt was “fat cats and an elegant font.” The result did not create full cat characters; it turned the clock numbers into plump cat-like shapes. That makes the feature feel more playful than essential, but it fits the broader push to place generative tools on smaller screens.
Breathing alerts become the highest-stakes feature
The most consequential new health feature is Breathing Emergency Detection. It builds on last year’s Loss of Pulse feature and focuses on severe drops in blood oxygen saturation, also called SpO2. If the watch detects a major drop, it can call emergency services and notify designated emergency contacts.
Google is trying to reduce false alarms by combining several signals. The watch uses heart rate, motion and altitude sensors as part of the decision. According to the demo information in the source article, the feature is not expected to trigger unless SpO2 falls to roughly 83 percent for an extended period.
The watch also disables the feature after it determines the wearer has been asleep for 30 minutes. That limit is meant to reduce false alarms tied to low SpO2 during sleep apnea or side sleeping. Google is positioning the feature around acute cases where someone may have stopped breathing, not as a tool for people with preexisting or chronic respiratory conditions.
Availability will vary by region. Breathing Emergency Detection will be available on both the Pixel Watch 4 and 5 in Europe at launch. In the US, it is pending clearance from the FDA.
Monthly trends aim to flag health changes earlier
Google is also adding less urgent, longer-term health tracking. The Pixel Watch and Fitbit Air will track nightly sleep breathing quality and provide monthly updates. The goal is to show how daily patterns may relate to respiratory health during sleep over a longer period.
Two other additions are Blood Pressure Trends and Insulin Resistance Trends. Google describes both as wellness features, not diagnostic tools. They require at least a week of data and are designed to flag monthly trends, more like a “check engine light” than a medical reading.
These features will not arrive at the Pixel Watch 5 launch. Their rollout is currently scheduled for September. They are also planned for the Pixel Watch 3 or later, as well as the Fitbit Air.
Blood Pressure Trends uses the optical heart rate sensor and motion sensors to monitor blood flow. That data is processed by an AI model that estimates the shape and speed of pulse waves under the skin. The result is not a systolic or diastolic number; it is an indication of whether blood pressure appears to be trending higher or lower.
Insulin Resistance Trends is the more unusual feature. It passively estimates whether insulin resistance is trending higher or lower month to month using signals such as heart rhythm, sleep quality and daily movements. Google says it tested the algorithm against blood draws and published findings in a peer-reviewed study in Nature.
Google is not presenting this as noninvasive blood glucose monitoring. It is also not a diabetes screening feature. The idea is to flag sustained metabolic stress over a monthlong period, which could make the watch more useful as an early signal device rather than a source of direct medical measurements.
Fitness tracking gets attention too
The Pixel Watch 5 update is not limited to health alerts and AI assistance. Fitness tracking is also part of the refresh, especially around GPS performance. Pixel Watch and Fitbit users have complained for years about unreliable GPS maps from outdoor activities, particularly in urban areas.
Google introduced dual-frequency GPS last year. This year, it is making stronger claims about GPS performance and better maps for workouts. The source article does not provide the full comparison claim, but the direction is clear: Google wants outdoor activity tracking to feel more dependable.
Taken together, the Pixel Watch 5 looks like a continuation of the strategy Google introduced with Fitbit Air in May. The watch is still a familiar piece of hardware, but its value increasingly depends on software that interprets context, health signals and movement patterns.
That also makes the limits important. Several of the most notable features are not diagnostic. Some are not available at launch. One is still pending FDA clearance in the US.
